Like other important life decisions, transitioning to residential care is more manageable when you have all the necessary information.

Our desire is for the process of entering Aged Care to be as simple and smooth for you as possible, as our role is to help support you and your family during this time of transition.

Before applying for a Government care subsidy, a few things are required of you. Please follow these three steps: 

  1. A health assessment (ACAT/ACCR) by an authorised and qualified practitioner
  2. An Assets Declaration from Centrelink
  3. A completed Pre-Entry Pack

10 Steps to Enter Aged Care

  1. Complete ACCR (ACAT) through your GP
  2. Centrelink Assets Declaration received by Adventist Care
  3. Complete Pre-Entry Pack
  4. Submit documents from Steps 1-3 to Adventist Care
  5. Pre-Entry Interview with Admissions Officer
  6. Wait for bed vacancy (You will be notified)
  7. Inform Adventist Care of any care need changes
  8. Receive Resident Handbook
  9. Complete Admission Pack
  10. Admission to Adventist Care

Pre-admission Checklist

Use this list to ensure you are adequately prepared to enter residential care:

  • Check if your GP will visit residents at Adventist Care in Rossmoyne.
  • Label clothing with the resident’s name.
  • Personal items, such as glasses, hearing aids, and dentures, are labelled with the resident’s name.
  • Seriously consider establishing an Enduring Power of Attorney
  • Seriously consider establishing an Enduring Power of Guardianship
  • Seriously consider establishing an Advance Health Directive

Prospective residents with their families are encouraged to visit the facility and meet with the Admissions Officer before placing their names on the waiting list.
